The Home & Community-Based Services (HCS) Program provides community-based services and support to eligible persons as an alternative to the Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID).
In order for a person to be eligible for the HCS Program, the following must be met:
- The individual’s county of residence is in the provider’s geographic catchment area or, if the consumer has been removed from his/her home community due to long-term institutional placement. He/she may be considered a candidate for the HCS program even though the program site is not located in the original county of residence.
- The individual must be eligible for the Medicaid Program and SSI. This will also include applicants who were once eligible for the received SSI benefits and continue to be eligible for Medicaid as a result of protective coverage mandated by federal law. Applicants who are under age 18 must meet the SSI criteria for disability and financial eligibility (i.e., institutional deeming and income resources).
- All program participants must meet the level of care criteria for an Intermediate Care Facility for Indiciduals with Intellectual Disabilities.
- The individual must have an Individual Plan of Care (IPC) for HCS developed by the Special Planning Team (SPT). The team must be composed of, at a minimum, the consumer, legal guardian and/or LAR, Service Coordinator, and the provider representative.
